Payments are now accepted through Square and Cash App.
Acupuncture and related services are provided and billed by Longevity & Wellness
Acupuncture LLC. All administered services must be paid for at the time of
treatment.
Please call your insurance company to see if acupuncture services are covered by
your plan. Longevity & Wellness Acupuncture will work with you to have your
treatments paid for by your insurance. We will provide you with a form, master bill,
that you can submit to your to your insurance company, and instruct them to send
the reimbursement directly to you.
If you need to cancel your appointment please do so 24 hours in advance. $30 fee
will be charged for cancelled appointments less then 24 hours or no-shows without
notifying the office in advance.
Late for your appointment
Late 15 min or more for an appointment may not be treated or you may receive and
abbreviated treatment within the time constraints of your originally scheduled
appointment.
There will be a $30 fee for each bounced check.
